ADDING THIS SERVICE AUTHORIZES THE BANK TO DISCONTINUE THE DELIVERY OF PAPER STATEMENT VIA U.S. POSTAL SERVICE FOR THIS ACCOUNT UNTIL WRITTEN CANCELLATION IS RECEIVED.

By checking the box below, I:

• Agree to keep the bank informed of any email address change. Allow 7 business days for your new email address to be updated within the system.

• Agree to access to the Internet through an Internet Service Provider selected by you.

• Agree to use an Internet browser consisting of one of the following programs: Microsoft Internet Explorer 8.0 (or higher), Firefox 3.5 (or higher), Safari 4.0 (or higher), Opera 10.0 (or higher), Google Chrome 8.0 (or higher. You can only access your accounts online with an internet browser that uses 128-bit encryption.

• Understand that I'll be notified by email that my statement is ready and will be available by logging into Web Banking.

• Understand that the bank will attempt to send e-Statements to this e-mail address one time per statement cycle and if the message is returned as 'undeliverable', that monthly statement will be mailed to me via U.S. Postal Service and my account will be reverted back to paper statements.

• Agree that I have access to the World Wide Web to utilize Web Banking through Chemung Canal Trust Company or Chemung Canal Trust Company DBA Capital Bank, a Division of Chemung Canal Trust Company.

• Understand that I must notify the bank in writing at: Attn. Deposit Operations Department, Chemung Canal Trust Company, PO Box 1522, Elmira, NY 14902-1522 or e-mail [email protected] to remove this service. No fee will be imposed to remove this service.

• Agree that e-Statements satisfy any and all requirements for the bank to send periodic written statements or notices for this account.

• Understand that I may request a paper copy of a statement or notice, which I have received electronically, by contacting 1-800-836-3711 or 607-737-3711 and that I may be charged a reasonable service fee for the preparation of this paper copy.
